Service Description

Some forms may require an accompanied appointments. $35 processing fee plus $5 each additional page for existing forms. Examples of new to patient forms: ESA Letter $200.00 We can assess you to see if your condition qualifies you to have an Emotional Support Animal (ESA). There are many laws governing this area of mental health services. Depending on your specific situation, you may be required to provide a letter from a mental health provider, but also other additional forms may need to be filled out. Discuss your specific situation with one of our team members. Gender Affirming Care Letter $200.00 We can assess you to see if your condition qualifies you to have a Gender Affirming Care (GAC) World Professional Association for Transgender Health (WPATH) service. There are many entities overseeing this area of mental health services. Depending on your specific situation, you may be required to provide letters from a mental health provider, but also other additional forms may need to be filled out. Discuss your specific situation with one of our team members Court Reports starting at $500.00 If you need an extensive report for court purposes (i.e., immigration, family), you came to the right place! Many of our providers already work with the court system and can provide various reports. Cost may vary depending on the time involved and the type of report that you need. Please contact our team for more specific information.


Cancellation Policy

Cancelation Policy 72-hour Reschedule (1-Week Cancellation) Policy Last minute cancellations/rescheduled appointments and "No-show" appointments have a negative impact on a business, including lost wages for our staff. In addition, when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill the appointment and patients on our waitlist miss the opportunity to receive services they need. For this reason and out of common courtesy to our providers, Psyched has the following Cancellation and Reschedule Policy: A non-refundable deposit is due at the time of booking. The remaining service fee will be charged one week prior to the service date. Once processed, these payments are non-refundable. Psyched understands things happen, thus if you need to reschedule, be sure to address this more than 72-hours (6:59 PM EST) prior to the appointment date and this can be credited towards any future service(s) executed within 90-days of the original appointment date. Credits are not guaranteed for cancelled or rescheduled appointments less than 72-hours prior to the appointment date and will not be arranged for any “no-show” appointment. For services with variable pricing, the lowest price will apply, i.e. 20 unit minimum for Botox, smallest syringe of filler or single vial for Kybella or sclerosant *these services are currently on hold. Excessive cancellations/rescheduled appointments may require full payment before appointments will be confirmed. Special discounts and pricing are not valid if this policy has been breached. By scheduling an appointment, you are agreeing to our cancellation policy. For those with unpredictable schedules, we do offer concierge appointments which can be requested through the contact page or email listed on our website. Your time is just as valuable to us as our own. As a courtesy, reminders will be sent to the email and phone number you provided when scheduling, however, if you do not receive a reminder, the above policy remains in place. We understand that emergencies can happen. Kindly notify us by email should an unforeseen event prevent you from keeping your scheduled appointment. Thank you, Psyched
